The SACCBC Client Portal provides our clients with secure and easy access to patient information, scheduling/rescheduling  appointments, filling out patient forms, and also viewing or paying invoices,  A link to our Client Portal will be provided via email to new clients.  To schedule an appointment with SACCBC, call 210.286.2586 or click here to visit Contact Page.

Client Intake Form

Prior to your first visit, we will need to collect some preliminary information about you and your needs.   The Client Intake form is available for completion on our Client Portal.  This form must be completed prior to your first visit.  To schedule an appointment with SACCBC and access the Client Intake Form, please call 210.286.2586 or click here to visit Contact Page.

Practice Policies/Counseling Agreement

Our Practice Policies and Counseling Agreement provides explanation of client confidentiality, responsibilities, fees, insurance claims, cancellations, rejected credit card charges, and acknowledgement and consent for counseling.  This document may be viewed here (click link above).  This document will need to be signed via our Client Portal. 

HIPAA Notice

The Health Insurance Portability and Accountability Act (HIPAA) explains the disclosure and use of your health information, your rights regarding your health information, and how to file a complaint if you feel your privacy rights have been violated.  This document may be viewed here (click link above) or via our Client Portal.
